Before you can execute the system, you must possess the tool. The Form FSH-1, Florida Student Homelessness Verification for Tuition and Fees Exemption Purposes is the official logbook that validates your status.

  • Action Command: Download the most recent version of the form from the official Florida Department of Education (FDOE) or your specific Florida College System institution's website (e.g., FSCJ, Valencia).

 

II. The System's Facts: Documenting Your Status

 

Your job is to provide clear, verifiable facts to your certifying official. The definition of "experiencing homelessness" is broader than just living on the street; it includes situations of unstable housing.

  • The System's Fact: You must confirm that you lack a fixed, regular, and adequate nighttime residence. This includes:

    • Sharing the housing of others due to economic hardship (doubled-up).

    • Living in motels, hotels, trailer parks, or campgrounds.

    • Living in emergency or transitional shelters.

    • Living in cars, parks, public spaces, abandoned buildings, or bus/train stations.

  • The Logbook Entry: You will need to provide your current contact information. If you have no mailing address, you must list the name, phone number, and mailing address of a reliable contact (e.g., a social service agency, general delivery address, or school address).

 

III. The Command: Securing the Institutional Signature (Part 1)

 

The goal of Part 1 is to obtain a single, authorized signature that immediately verifies your status, requiring no further action from the college. This signature is your system anchor.

Action Command: Target one of the following authorized individuals for the most efficient path to verification:

  • Shelter Director/Designee: A director of an emergency or transitional shelter, street outreach program, or other program serving individuals experiencing homelessness, or their designee.

  • School District Liaison: The designated Florida school district's liaison for homeless children and youths.

  • Continuum of Care (CoC) Lead Agency: The lead agency for the Continuum of Care, or its designee.

  • Federal Program Director: A director or designee of a program funded under Federal TRIO or GEAR UP.

  • Financial Aid Administrator (FAA): A FAA at the current institution, or another institution who previously made a determination.

The Chaplain's Decree: Choose the individual with whom you have the strongest current connection. This person has the authority to verify your status based on your circumstances, transforming your verbal claim into official, verifiable documentation
